BURSARY APPLICATION FORM FOR SPEECH AND LANGUAGE THERAPY

This form is to be completed by the parent, caregiver, teacher, or health professional, to support the application process for entry to the Finding Charlie’s Voice lottery scheme for financial assistance to support the costs associated with speech and language therapy/assessments or assistive technology. Finding Charlie’s Voice is providing a fund valued at €10,000 and will be awarding, on a random selection basis, 8 x €1,000 individual General SLT bursaries and 1 x €2,000 Rushbrooke Technology Grant.

This fund will open on March 19th and will close on April 16th. Any applications received after this date will not be considered for the fund. Should the fund be undersubscribed, a second call will be made and any applications received after 16th April, will be automatically included in the second call.

This form must be accompanied by the 'Bursary Referral Form for Speech and Language Therapy''.
